10 Essential Dental Notes Templates

1. Comprehensive Exam

The comprehensive exam template is suited for dental practitioners who wish to create detailed exam notes, especially for new patients. The clinician names and patient advice sections can be easily personalised.

This template meets the recommendations set out in Clinical Examination and Record-Keeping (FGDP). If you decide to further personalise the template using our Template Editor (Templates>Edit>'Template you would like to adjust'), don't forget to include 'Add Audit' to ensure that you are always recording compliant notes.

2. Basic Exam

Our basic exam template is still FGDP-compliant, but is more streamlined compared to the comprehensive version. Perfect for those in dental practice who wish to make accurate patient records quickly. This can be particularly useful for recall appointments.

3. Emergency

This clinical note template allows users to record the most common dental emergencies from start to finish, beginning with the patient complaint, through to the examination, special tests, diagnosis and treatment. This allows for easy record keeping in just a few clicks. As there are a limited number of diagnoses (and associated treatment plans) that occur commonly within an emergency appointment, these are all built into the template so you can click through easily.

4. Filling

Our filling template has been optimised to make sure users can make detailed notes for any type of filling. Valid consent is ensured as risks are specific to the scenario. For example, the risk of needing root canal treatment or extraction is only output as a linked note if the filling is particularly deep.

5. Extraction

Is simply writing 'risks and warnings given' enough? According to dental indemnity companies, this is not the case. All dental extractions come with risks, which need to be explained to the patient and which form part of the consent process (along with discussion of the benefits of the procedure and alternative treatment options including no treatment).

Using this stock template will make sure that the risks mentioned are unique to the particular tooth that is being extracted and ensure accurate records. In this way, using Kiroku can help protect the dentist from litigation as well as provide the highest standard of patient care.

6. Invisalign Assessment

7. Hygiene Checkup and Treatment

This clinical note template is based on guidance provided in the 'Healthy Gums Do Matter Toolkit', which is supported by the British Society of Periodontology. 8. Implant Assessment

Our implant assessment Note and Doc templates, developed in partnership with Dr Martin Wanendeya, can be edited to reflect your own workflow, for example with details of the particular implant system that you use. Full consent is captured in the patient notes, including alternative treatment options for space replacement.

9. Root Canal Single Visit

Carrying out dental treatment usually involves the same sequence of steps in the same order each time. This root canal treatment template incorporates commonly-used file systems, gives a range of average canal lengths for each root for each tooth, and common materials to make sure comprehensive notes are recorded.

10. Child exam

Like our adult exam template, this template has been developed in line with current guidelines and in a way that makes sure that all relevant information is captured for children of all ages and of different levels of caries risk.
